Self Referral Form

Health in Mind is a service offering support to adults experiencing mild to moderate anxiety or low mood living within the East Sussex area (excluding Brighton and Hove).

If you are feeling stressed or down due to life events such as unemployment, a relationship break-up or financial difficulties (to name but a few) we can provide help and support. If you have particularly severe difficulties, or if your difficulties have been going on for some time, you should contact your GP so he or she can ensure you get the help you need.

In order to help us make the best decision about what support would benefit you the most now, we need to gather some important information about you, your difficulties and how you are currently feeling. Please answer all the questions in the self-referral form below.

The information you provide will remain confidential, unless we have concerns about your or another person’s safety.

Are you registered with a doctor's surgery which is NOT in the county of East Sussex? If you are not a resident of East Sussex and live elsewhere, you may not be eligible to access our service. As we are part of a national programme of talking therapies services there will be a service near you. Please Click here to find your local service. If you are a student in East Sussex but your doctor's surgery is out of this area, you are welcome to continue your referral to us but you may instead wish to contact the local NHS talking therapy service which is joined to your doctors using the link above.

Are you currently being seen by another mental health team? If you are currently receiving counselling, being supported by a community-based team, seeing a psychiatrist or receiving long term mental health support, Health in Mind may not be able to support you at this time. We advise to speak with your care-coordinator or your regular appointed support person.

We will ask you more about this in the referral form so we can consider if we are the right service for you at this time. If there is a service which is more beneficial for your needs we will let you know.

Health in Mind is not a crisis or emergency service and cannot provide an urgent response

You can get help with your mental health in different ways depending on what you prefer. All services are free.
