We are here to help, all you need to do is fill in a simple form.

If you have any difficulties completing the form, please call us on 0207 683 4278.

If you are in a Mental Health crisis, please note that this is not an emergency or urgent care service.

If you need urgent help and are worried that you can't keep yourself safe, please call 111 and select option 2 for mental health support or you can get help at your local Accident and Emergency department.
