To refer to NHS Buckinghamshire Talking Therapies please complete the information below. It is important that your patient understands and accepts our terms and conditions at the bottom of the form for your referral to be processed. This information is to help us provide the best possible care for your patient.

At the end of the form you will be presented with a range of possible telephone appointment slots to choose and book for your patient. Only do this if your patient is with you and can confirm that they are able to attend that appointment. If you cannot find any suitable slots, they require an interpreter, or no slots are available, please complete and submit the self-referral form without an appointment. We will then endeavour to contact the patient as soon as possible to offer them an appointment (or they can contact us to book an appointment on 01865 901600).

Terms and Conditions

To process the referral we do need to ensure that your patient understands and accepts our terms and conditions of how we look after their records, the main points are:

  • We share information about your patient's care with their GP
  • We share information with the Department of Health; this is for statistical purposes only and will not disclose your patient's identity
  • Further Information can be found in the above link to ‘My patient accepts the terms and conditions’ (which will also be sent in the confirmation email).

What will happen next

If you manage to choose and book a telephone appointment for your patient at the end of this self-referral process, they will be contacted at the agreed time on the telephone number you provided to talk to your patient about how they are feeling, and explain the different types of therapies we offer, and agree with them the best next step.

If you have not managed to find a suitable appointment slot, please submit their referral and allow two working days for us to process the information. If we think we may be able to help them, we will usually offer you a telephone assessment appointment with one of our team. If we think that they could benefit from a different service, we will contact you (and your patient) to let you know.
