Welcome to NHS Oxfordshire Talking Therapies service
Our service is open: Monday 8am-5pm, Tuesday to Thursday 8am-8pm and Friday 8am-5pm.

To refer to NHS Oxfordshire Talking Therapies please can you complete the information below along with two other brief questionnaires. It is important that you read and accept the terms and conditions at the bottom of the form for your referral to be processed. This information is to help us provide the best possible care for you.

This service is only for 18+ age.

After we receive your self-referral we will contact you via email within a couple of days with further information. Therefore please provide an email address.

The numbered coloured boxes above show the names of the various forms to be completed in order to refer yourself. Each one is a necessary part of the referral process and provides us with important information. The entire referral process should take around 15 minutes to complete, depending on the detail of your responses.

SilverCloud is an online treatment option. If your referral data indicates that SilverCloud is an appropriate treatment for your needs then it is made available to you via an email direct from the SilverCloud Service.

SilverCloud is only available in English. If you require an interpreter alternative treatment is available and you can access this by referring online at NHS Oxfordshire Talking Therapies.

By completing this referral form you are agreeing for some basic information to be sent to SilverCloud in order to create an account with them.
