Patient Portal improving patient wellbeing
We provide support to people aged 18 or above who are registered with a Manchester GP. Please note, we require consent from the client to process the referral, if the client has not given you permission to submit this referral, do not submit the referral. We provide support for common mental health conditions, such as anxiety and depression, including: counselling, cognitive behavioural therapy, mindfulness, or guided online therapy.The service is delivered by a range of professionals including psychological wellbeing practitioners (PWP`s) and counsellors, who will find the support that is right for the person. This could be a group workshop, an online or telephone based course of self-help treatment, or an individual therapy session.If, for any reason, our service isn`t right for the person, we will make sure that they see the right professional for their situation and get the support they need with the community.To help us put the person in touch with the team who can give them the support they need, please complete the form below.Please note: this service is not a crisis service and will not be suitable if the person is at immediate or imminent risk to themselves or others.For those who feel that they are in distress, for example if they feel at risk of harming themselves or others then we would urge them to contact any of the following organisations who are able to offer support: NHS (non-emergency) 111 (24hrs) Samaritans (Helpline)116 123 (24hrs) Alternatively, we would request that they contact their GP.
