Action for Children is an emotional wellbeing service and you understand by accepting our service you give consent to:

Information being collected about you and your family
Relevant agencies may be contacted in relation to your referral so that we can develop and implement a support plan.
Information may be shared with relevant agencies to evaluate the effectiveness of Action for Children.
The sharing of any information with outside agencies will be carried out in accordance with Data Protection Legislation.

The information you supply to Action for Children may be shared with Staffordshire County Council and their partners if they supply a service to you. These partners can include Health, Schools, Voluntary sector, department for work and pensions and the Police. This is to ensure that youreceive the best service available and improve communication betweenorganisations. Staffordshire County Council will not disclose yourinformation to any partner who is not supplying a service to you unless required by law.
